The Reset Clinic: Diabetes Remission Shared Medical Appointments

Hosted at Prisma Stone & Main Primary Care. This small-group, physician-led program is designed to help individuals improve blood sugars, reduce medications, and work toward diabetes remission through evidence-based Lifestyle Medicine.

About
This program is fundamentally different from the traditional approach to “controlling” diabetes. Rather than simply managing blood sugars with medications or avoiding carbohydrates to work around insulin resistance, this model focuses on addressing the root cause: insulin resistance itself. We do this through an evidence-based, plant-forward approach, aligned with the recommendations of the American College of Lifestyle Medicine. The goal is not better numbers; it’s getting diabetes off your diagnosis list.

Program Format
• Shared Medical Appointments with 8–12 participants
75-minute visits including medical care, education, and group discussion
• Led by Dr. Beth Motley

Schedule
Meets every two weeks for six sessions: Jan 15 & 29, Feb 12 & 26, Mar 12 & 26
Two options each date: 8:30–9:45 AM or 10:15–11:30 AM

Eligibility
Current diagnosis of type 2 diabetes. Established with any Prisma primary care provider and last visit within in the past 12 months. Active MyChart account for eCheck-In prior to appointments. Willing to participate in a shared-visit format.

Billing
These are billed as standard office visits through your insurance, just like when you go in for an a1c check and have medication adjusted.

Join Beth Motley MD FACLM, Family Medicine/Lifestyle Medicine physician, to learn how to implement Lifestyle Medicine in your practice. It can be done, with ease and while maintaining productivity, and it’s a truly rewarding way to practice. She will go through how her office visits typically run, review key points for motivating patients, and make all of her materials available to you. Students, residents, and faculty are invited to attend.

We will discuss key dietary components of a heart-healthy, plant-based eating pattern and how they impact inflammation, lipid profiles, endothelial function, and overall cardiovascular risk. And finally, we will explore practical strategies to counsel patients on adopting sustainable plant-forward dietary changes within the context of preventive cardiology nursing care.
